Recovery Connect Somerset

Through this program, participants receive individualized assistance to access peer recovery support, employment readiness, and job placement, housing stability, transportation, family counseling, relapse prevention, and other wraparound supports that promote sustained recovery and community reintegration. Program Components include:

Referral Coordination: Empower accepts referrals directly from Recovery Court case managers and probation officers. This system helps ensure timely and coordinated support for individuals in need of recovery services. 

Comprehensive Recovery Planning: Each participant works with a Certified Peer Recovery Specialist (CPRS) to develop an individualized recovery plan addressing housing stability, employment readiness, transportation, childcare, behavioral health needs, education or training, and relapse prevention.

Peer Recovery Support: CPRS staff provides ongoing mentorship, advocacy, and non-clinical support, as well as supports participants as they navigate recovery and resources.  

Supportive Service Assistance: Each participant is eligible for a flexible supportive-services stipend to address immediate barriers. For example, housing, transportation, childcare, utilities, or workforce-related expenses. This financial support helps stabilize participants across the Four Dimensions of Recovery and reduces obstacles to treatment engagement and community reintegration. 

Resource Linkage: Participants are connected to local community supports, including sober living houses, behavioral health services, transportation assistance, and family counseling.

Follow-Up and Retention: Empower conducts follow-up assessments every three months. The assessment monitors progress and measures changes in recovery capital through the Recovery Capital Scale (RCS). Services will be adjusted based on emerging needs. 

Community Outreach and Education: Program staff collaborates with community partners to increase awareness of available recovery resources, promote cross-system coordination, and strengthen navigation across Somerset County’s recovery ecosystem.
